New Portal—Elation “Passport”—Coming Jan. 17, 2020

As many members experienced these past few months, the Patient Fusion portal for messaging our providers/staff has not functioned acceptably. Those challenges, along with a desire for enhanced functionality, prompted us to switch to a new, improved Electronic Medical Records (EMR) software with a secure portal called “Passport.”

What this means for you: 

Friday, January 17: Members will receive an email inviting them to register for “Passport.” the new portal. Follow the simple instructions to create your new account. You can even download a handy app!
Friday, January 24 @ 12:00 pm: Patients can no longer send messages in the old portal, Patient Fusion. *Data will be visible through Jan. 29
Thursday, January 30: Patients can no longer log in to the old portal. *Prior to this date, please review and save any correspondence/lab copies you wish to keep.

 

Accessing Your Medical Records (through Jan. 29, 2020)

You may access some of your medical records in our electronic personal health record called Patient Fusion. You will need the access code which was sent to the email address listed when you established care. If you have not received an email with this information, please contact us and we will activate your patient portal.

 

If you have lost your login or password, and are having difficulty resetting your access, please watch the following video. The process is multi-step because of the security required for medical information.
