Elation’s “Passport” is Your New Patient Portal!

As of Friday, January 25, 2020, we have emailed all active, established patients with an email address and mobile phone number on file an invitation prompting them to enroll in our new patient portal: Elation’s “Passport.” 

 

  • If you have not located this email in your inbox, please check your Spam/Promotions/other folders.
  • In the invitation email, simply select the “Click here to Register for Passport” button to be automatically texted your one-time security code and taken to the login page.
  • *If for any reason you do not receive the text mentioned above, simply call our office so we can provide that code to you.

We encourage you to enroll in Passport as soon as possible for this convenient way to contact your provider and vice versa. Thanks to those of you who have already done this!

The old patient portal, Patient Fusion, is no longer active. If you are a former member who needs copies of your medical records, your new provider may submit that request to us directly via fax at 828-575-2298, or you may request them by emailing [email protected]
