Convenient access to your medical provider through your Passport Patient Portal

Members receive an invitation to enroll in the Passport patient portal upon completing their first clinic visit. After receiving the invitation, please enroll promptly to gain access to important medical records (lab results, visit notes, etc.) and conveniently, securely communicate with your provider. *Passport replies and refill requests may take up to three business days to process.
  • When you receive the email invitation to the patient portal, simply select the “Click here to Register for Passport” button to be automatically texted your one-time security code and taken to the login page.
  • If for some reason you can/do not receive the text noted above, please call our office so we that can provide you with the required code.
  • Please check your Spam/Promotions/other folders if you are unable to find the email invitation.
