If you have potential COVID-19/Flu symptoms—recent onset of fever, cough, shortness of breath, loss of smell, sore throat, fatigue, loss of taste, sore muscles, runny nose, headache, or diarrhea—you must be seen first either curbside or virtually or have a *clinic-verified, negative COVID test, regardless of vaccine status. *Performed within 24 hours by a medical professional.

*Annual visits must be booked with your current primary provider. Those wishing to switch to another provider must call our office to request approval since some providers have full panels. New members must complete an Establish Care visit first; Annual visits are for renewals only.  *Annual visits are in-person so that we can perform a comprehensive physical at least once/year.
